Instructions

Please complete the electronic registration form below. An email will then be sent to the Head of Household's email address confirming your submission.

• Please be sure to include dates and places of Sacraments received. Exact dates are not required, but a rough estimate of a year is most appreciated.
• The name of the Church, City and State where Sacraments were received is required for each family member in order to complete your registration.
• Please also include all requested information for members of your family who are not Catholic. The only sacramental information required for non-Catholic family members is the date and place of his/her Baptism. (If never baptized, please indicate “N/A” for “not applicable”.)
• Any questions or clarification you feel is important can be made in the Notes section at the bottom of this electronic form.
• Items with an astrisk (*) are required in order to submit the form.

• Once your information is received, we will complete your registration and notify the Diocese of Harrisburg that you have registered in our Parish.
