• Youth Protection Background Checks - Diocesan Disclosure Form     -     Child Abuse History Clearance
• AU policy signature form for K-6  Also gives permission for publishing, use of photographs
• AU policy signature form for PRE-K.  Gives permission for video and photograph use for PRE-K

 

Miscellaneous Downloads - the following are miscellaneous downloads.

• TEACHER Identifiable Photograph Permission Form - if you wish to have an identifiable photograph image on this website, download this form, complete it, and give it to the technology department.

• Microsoft PowerPoint Viewer from Microsoft.com- Once downloaded, you can view Microsoft PowerPoint presentations without having to have PowerPoint itself installed on your computer.
• Microsoft Excel Viewer from Microsoft.com- Once downloaded, you can view Microsoft Excel spreadsheetswithout having to have Excel itself installed on your computer.
• Microsoft Word Viewer from Microsoft.com- Once downloaded, you can view Microsoft Word documents without having to have Word itself installed on your computer.
• Microsoft Windows Media Player from Microsoft.com- Once downloaded, you will be able to view a variety of different movie formats and listen to a variety of music formats.
• Hyperstudio Player from Hyperstudio.com- Once downloaded, you can view Sunburst Hyperstudio files without having to have Hyperstudio itself installed on your computer.
• Adobe Flash Player from Adobe.com- Allows surfers to view enhanced websites.
